The Banyan, Changing the face of Mental Health across India, India
The Banyan serves individuals with mental illness from lower social economic groups. Major components of the program include providing early and crisis intervention in homeless shelters and outpatient facilities, spreading awareness in the community, and developing these experiences into models that can be offered to other stakeholders for replication.

HCF and Healthways Australia, MyHealthGuardian, Australia
MyHealthGuardian offers advice and support via two integrated services—one for improving overall well-being and the other for managing chronic disease, which provides behavioral change programs to help people make positive lifestyle changes and individualized help to manage chronic illnesses. The program has successfully reduced hospitalization rates and improved participants' quality of life.

International Virtual e-Hospital Foundation, Initiate-Build-Operate-Transfer: A Strategy for Establishing Sustainable Tele-Health, United States
The Initiate-Build-Operate-Transfer: A Strategy for Establishing Sustainable Tele-Health program aims to create self-sustainable international telemedicine and e-health programs and to rebuild medical systems in the Balkans and elsewhere in the developing world. Based in Alaska, IVeH uses telemedicine and collaboration to educate healthcare providers about the implementation of telemedicine, e-health and electronic libraries.

Lembaga Pembina Anak Bangsa (LPAB), Anti-Smoking to Children Under 18, Indonesia
LPAB promotes non-smoking and healthy living to children under the age of 18 in destitute communities in Indonesia. It combines discussions with children regarding how to stop smoking; obtaining clinical data and support from community leaders and organizations; and providing medical examination, immunization, and proper nutrition to set these children on a healthy path of life.

Riders for Health, Building a Healthier Future through Reliable Transportation, Africa
Riders for Health is a social enterprise that aims to increase access to health resources in sub-Saharan Africa. The Building a Healthier Future through Reliable Transportation program sustains vehicle performance and works in partnership with health-focused organizations to ensure increased and reliable delivery of essential healthcare services to communities otherwise out of reach. Its sustainable model is embodied in its local management and environmental focus.

University of Miami, Florida Telemedicine Trauma Network, United States
The goal of the Florida Telemedicine Trauma Network is to establish electronic connectivity between trauma centers and remote sites such as community hospitals to facilitate the sharing of trauma expertise for the effective treatment of patients, extend the reach of trauma education, and strengthen disaster response and preparedness in case of a public health incident.

Apparel Lesotho Alliance to Fight AIDS, ALAFA, Lesotho
Launched as a response by Lesotho's textile and apparel industry to the increasing impact of HIV, the ALAFA initiative, in partnership with factory management, doctors, and the Ministry of Health and Social Welfare, has established programs to minimize the impact of HIV and AIDS by providing HIV and TB prevention and care for apparel employees and their spouses or partners.

BJC HealthCare, Help for Your Health, United States
Help for Your Health offers its employees opportunities to live healthier, such as free employee health fairs, health education, healthier cafeteria fare, and access and/or discounts to recreation/fitness centers, sports equipment, healthy food and weight loss programs. The program follows a "Super Six" health focus: blood pressure, cholesterol levels, BMI, diabetes management, tobacco use and the use of a primary care physician (PCP).

Dallas/Fort Worth International Airport, LiveWell Program, United States
LiveWell aims to change the culture of DFW Airport and encourage employees to lead a healthier lifestyle. Through its Incentive Policy, which allows participating employees to earn extra pay through completion of health assessments, educational training, and nutritional or fitness-based challenges, it has helped reduce health care costs, sick days, and absenteeism.

Hao2.eu provides content and services using the latest 3D virtual world to promote education, wellbeing, and access to services to meet the needs of people with disabilities, especially Autistic Spectrum Conditions (ASC), and their support network. 80% of Hao2.eu employees have ASC, and their direction is central to creating sustainable improvements to workplace inclusion and community health and wellbeing.

State of Rhode Island, Office of Employee Benefits, Rewards for Wellness, United States
This program was established by the Rhode Island state government to encourage long-term employee lifestyle changes and improved health and wellbeing through wellness programs and screenings. The program ties beneficial lifestyle choices made through the program to financial savings, awarding participating employees with health insurance premium co-share credits.

Universidade Federal de Minas Gerais, Telecare to Remote Areas, Brazil
Telehealth supports remote municipalities in Brazil, allowing access to specialized medical services by low-income populations located in remote areas. The program allows health professionals in geographically isolated areas, with little access to other specialized professionals and without technological resources, to keep abreast with current techniques and diagnose and treat certain diseases or urgent cases.
